I heard somewhere it was a targeting glitch. Still. The second point stands true with OpenAI recently

There’s no evidence it was ai at this time, it was purely human error. Per the NYT (which all the other news agencies are quoting from too)

Kinda a glitch, it coded the school as still part of the military complex based off outdated information from prior to 2016. But not ai being stupid glitch
