Just be straight forward with her, you just don’t have to be a douche about it. Tell her you are unhappy in the relationship and don’t believe any changes she could make would make a difference in your decision.

I think just like that. Or close to that. Tell her how you feel (unhappy). You don’t need to defend what you want or your decision any more than that.
