My grandfather insists he is fine but is acting very confused and disoriented and I feel like I should call an ambulance but I don’t know and I know he’d get really upset at me for it

Definitely call and a non-emergency line to report his confusion and disorientation, and get advice on potential stroke signs. try a local ER phone number, local fire department, or even his doctor’s office if you know it. can you drive? tell him to go with you to the hospital voluntarily and if he refuses then call an ambulance i suppose

who cares if he gets upset, i’d rather he be upset at your overreaction than something is wrong and the underreaction was the problem. l
