Why don’t doctors even quickly scroll through your chart before/during your appointment? (elaboration in comments)

I don’t expect my PCP to remember me as I only go in at max every other month but usually it’s every 3 months. But here’s an example: I went in today to follow up on labs and I wanted to make sure a symptom is likely caused by my severe vit D deficiency. She says “hmm I don’t even think we tested for that…” and I have to tell her that not only was it tested for but that she prescribed me like the highest possible dose of vit D to take.

It’s really frustrating when it’s obvious they never looked at your chart. Like usually there is some sort of symptom that led to a referral, you’d think they look. I don’t understand what happened to preparing for appointments. Patient care is just really declining, for so many reasons

The only dr that I didn’t experience this with is my GI. Honestly he was a godsend, and he actually takes my concerns seriously. Cause he actually looked at some of my past dr notes and agreed that there is most likely an underlying issue that hasn’t been addressed yet. I also had a nurse who went and did research (in her own free time) and suggested I get tested for something that I had actually considered asking about at one point

and then regarding my symptoms she asked if I had any family history of autoimmune disease as that could be a cause. And I had to say “Yes… me. I have an autoimmune disorder. So does my mom, and my mom’s mom, etc.” But I am literally on meds *for* this and it’s in my chart.. Like I said I do NOT expect her to remember me AND I get the merit of hearing history from the patient but I feel like they should at least skim your chart? Especially since she was prescribing me stuff, what if it—

—interacted with another of my meds? BTW, This is a *good* doctor. This is not specific to her. In my entire life I have never had a doctor that actually remembered or looked at my chart, it’s always like this. I feel like I could say anything about my health history and they’d have no clue if it’s true. But she’s a good doctor. She actually does listen and care, doesn’t dismiss me, etc. First one I’ve seen like that!
