first doesnt hurt as much but itll hurt right after (my experience), try not to suddenly jolt/move bc of the pain, if anything weird happens with the piercer/ they make you feel uncomfortable pls leave!! also bringing a friend can sometimes make u more comfortable, some places let u do a numbing gel but thats up to u and i havent used it before but ive heard mixed reviews
