PCOS typically causes visible symptoms (increased amount of facial hair, acne, weight gain etc) and any sign of poor health is considered unattractive from an evolutionary standpoint

Not a turnoff for me. However if she doesn’t work to manage it it would be (not physically I mean like mood swings and depression)

except it's not something we can control and it's also fairly easy to manage, and not all women with PCOS will have those symptoms bc you can get on medication for all them.
